Rolul ecografiei transtoracice in diagnosticul infectiilor fungice invazive la pacientul cu leucemie acuta

REZUMAT

Pacienţii cu leucemie acută, categoria cu riscul cel mai mare de a dezvolta infecţii fungice invazive, prezintă rate înalte de morbiditate şi mortalitate cauzate de aceste complicaţii. Diagnosticul acestor infecţii
trebuie să fie exact şi cât mai precoce. În acest sens, metode non-invazive, rapide, disponibile permanent şi cu cost redus sunt căutate. Diagnosticul imagistic are o importanţă majoră, examenul computer tomograf
fiind standardul de aur. Ecografia transtoracică întruneşte multe dintre condiţiile necesare, oferă informaţii valoroase şi perfect superpozabile examenului computer tomograf toracic. Iniţierea tratamentului antifungic are rezultate net superioare faţă de terapia empirică, ameliorând rata de supravieţuire a acestor pacienţi.

De novo hepatitis B infection in a HBsAg-negative patient with chronic lymphoproliferative disorder

ABSTRACT

HBV reactivation is becoming an important cause of morbidity and mortality in patients with hematologic malignancies undergoing chemotherapy, at highest risk being HBsAg positive patients (active or inactive chronic HBV infection). The occult carriers of HBV have a lower risk, but recent data reveal that reactivation risk is increasing and complications like fulminant hepatic failure and even death can occur. Pre-emptive antiviral therapy is the current standard for HBs Ag positive patients, but for occult carriers routine antiviral prophylaxis is recommended only in certain circumstances.

We report a case of „de novo“ hepatitis B in a patient with non-Hodgkin’s lymphoma and an unknown occult carrier status, which experienced viral reactivation after 5 month of combined fludarabine therapy. Lamivudine was initiated when HBs Ag seroreversion occurred and the outcome was good; she was then able to continue chemotherapy with rituximab-based regimen, maintaining antiviral treatment.

ABSTRACT

We present the case of a patient diagnosed in SUUB Hematology with adult T-cell leukemia-lymphoma. The particularities of this case are the delay in diagnosis, the apparent onset of disease with severe eye and skin determination in the leukemic phase, and the particular lung involvement in the final stage of the disease.

Importanta clinica a modularii tratamentului cu Voriconazol în cazul aspergilozei pulmonare invazive pentru doua cazuri de leucemie acuta mieloblastica

REZUMAT

Introducere. Pacienţii cu leucemie acută reprezintă categoria de pacienţi cu neoplazii hematologice cu risc mare de dezvoltare a infecţiilor fungice invazive, iar agentul patogen frecvent implicat este Aspergillus spp.

Scop. Cele două cazuri prezentate îşi propun ilustrarea importanţei individualizării algoritmului de diagnostic şi tratament al infecţiilor fungice invazive la pacienţii cu leucemie acută pentru obţinerea unui răspuns optimal la tratamentul antifungic.

Material şi metodă. Prezentăm două cazuri de leucemie acută la pacienţi tineri, care dezvoltă aspergiloză pulmonară invazivă în cursul perioadei de neutropenie severă postchimioterapie. Diagnosticul se bazează pe criterii clinice, pe probe micologice pozitive, dar mai ales pe criterii imagistice, care au evidenţiat aspecte înalt sugestive pentru aspergiloză pulmonară invazivă şi astfel au determinat iniţierea promptă a tratamentului antifungic.

Concluzii. Cele două cazuri ilustrează evoluţia favorabilă a pacienţilor cu leucemie acută şi aspergiloză pulmonară invazivă în contextul tratamentului cu Voriconazol iniţiat prompt şi importanţa ajustării dozelor în funcţie de evoluţia clinică.
